Payment is due in full at time of service
Cash, Check, Credit Card and HSA card are accepted
A small number of sliding scale spots are reserved for those in need. Please contact me to learn more.
INSURANCE
While Perennial Health Acupuncture does not accept nor bill insurance, a receipt with billing codes on it (a “Superbill”) can be provided upon request, which you can submit directly to your insurance provider for reimbursement. This does not guarantee full or partial reimbursement. Fees reimbursed by insurance are dependent on your individual coverage.
CANCELLATION & RESCHEDULING
We realize that sometimes emergencies arise, and cancelling an appointment might be necessary. Perennial Health Acupuncture requires a minimum 24 hours notice for all cancellations. Appointment changes can be made quickly and easily online via your patient portal.
If you miss an appointment (“no-call, no-show”), or cancel with less than 24 hours notice, you will be billed for your full scheduled treatment cost. No exceptions will be made for COVID or illness related cancellations made with less than 24 hours notice. Exceptions may be be made for medical and family emergencies.
